Do you see any ios crash info in the flash ?
Have you tried rebooting the swith ?
if you reboot do you get the same message ?
can you provide the console logs if any and show controller o/p ?
show tech-support would be great :)

You mentioned above that PoE is working before, are you using the same 7960G IP phone? If yes how many?
By the time you encountered this problem, are you adding a PC or another IP phone?

What is the exact 3750 platform you are using? IOS?

Check also the show power inline output, to see if your switch have exceeded the maximum power (sum of power allocated to all switch ports) and check if your power supplies are healthy (seems you are using stack wise) is healthy.

HTH,

there is no ios crash info in the flash, Initially I use 'c3750-ipbase-mz.122-35.SE5.bin' then try upgrade to 'c3750-advipservicesk9-mz.122-37.SE.bin', restart/reboot and still get the same message.
from the console logs, its just detect the PC that directly connected to port 41,45,15 etc=

my c3750 is 48 port, I hv 12 cisco ip phones, 16 PCs and 3 servers.

HTH,

yes, 1 month only, 12 cisco ip phone 7960G.
no, I didn't add any pc or another ip phone.
now, I use = c3750-advipservicesk9-mz.122-37.SE.bin
the config is : 'power inline auto', then tried to configure manually 'power inline consumption <4000 - 15400 miliwatts>' but still failed.

strange, on last troubleshoot; I deleted the NVRAM config, reboot, unplug all cable except 1 ip phone.. and still same. btw is it the PoE use the specific hardware/module or some thing like IC/circuit inside the switch?? or if yes, is it possible my new 3750 switch damaged and just working 1 month..

Post here the "show power inline consumption" and "show power inline fx/x" (to where the IP phones are connected) output to check the power and PS status.

Did you try configuring "no power inline auto" and then issue "power inline consumption <4000 - 15400 mW>"?

it seems to be an hardware problem. I would ask a RMA if I were you (or open a support ticket at cisco)

thanks dreamer2062 for ur advice, I'll do it later; but I just want to make sure its not caused by misconfig or something else.

Seems HW failure to me too. Ask Cisco for an RMA. If you want to know the exact root cause of the HW failure, you may ask Cisco TAC for an EFA..failure analysis

or if you are having a STACK .. take it out and try to boot again.. if it booted wihtout any problems then try to configure the first switch as master
